Health Information Resources and Vocational
Resources provide a variety of case management activities for individual
clients with a wide range of disabilities.
Services include individualized assessment of injuries and long-term
disabilities with identification and coordination of medical, vocational and
rehabilitative services and life care planning, as well as expert
services. We provide evaluation of
the individual’s requirements from the time of injury or diagnosis with the
availability of continuous case management and follow-up.

Sharon L. Reavis, RN, MS, CRC, CCM
Medical Case Manager Life Care Planner
Sharon L. Reavis is a Registered Nurse and a Certified
Rehabilitation Counselor with extensive experience in Medical Case
Management and Life Care Planning. Ms. Reavis holds a Masters degree in
Rehabilitation Counseling. Ms. Reavis has taught at the graduate level and
continues to lecture on the topics of Case Management and Life Care
Planning. She is a Certified Case Manager and a Fellow of the International
Academy of Life Care Planning. Ms. Reavis has served on myriad boards and
committees in the fields of Nursing, Rehabilitation, and Life Care Planning,
and is an Editor for the Journal of Life Care Planning.
Peder K. Melberg, MA, CRC
Vocational Rehabilitation Counselor / Evaluator
Peder K. Melberg is a Vocational Rehabilitation Counselor
specializing in case management, rehabilitation counseling and vocational
evaluation. He provides vocational case management services to clients
throughout the Commonwealth of Virginia and surrounding states. Mr. Melberg
holds a Masters Degree in Rehabilitation Counseling from the University of
Wisconsin and is a Certified Rehabilitation Counselor. He is also certified
as a Diplomate by the American Board of Vocational Experts. |
